3/17/2024 EnchantaVaughn Approved Behavior: At the start of the session, Laylah was viewed as displeased, unreceptive, and short-tempered, as it was reported by the mother that Laylah was not having a good day. Laylah displayed angry outbursts and lashed out at QMHPA when inquired about why she was having a bad day at which mother reports these behaviors last for 10 minutes in duration. Behavior: Laylah avoids self-reflection exercise when QMHPA inquired her to deeply think about her reaction to certain situations and what she should do to avoid them. Laylah was confronted by QMHPA and asked why she made such a big fuss when she was in the wrong to begin with and when these behaviors are being presented, they last about 10-20 minutes in duration. Intervention: QMHPA will meet with Laylah daily to discuss progress and expectations that are working and not working in the home weekly. QMHPA actively listened to Laylah verbalize that she was agitated today and the reasons to her agitation. Instruction, modeling and role-playing techniques were used to help Laylahâs general communication skills. QMHPA taught Laylah effective problem-solving skills (i.e. identifying the problem, brainstorm alternate solutions, select an option, implement a course of action, and evaluate). QMHPA informed Laylah that when she is agitated to refrain from taking it out on people and communicate how she is feeling to others, so they know exactly how she is feeling. Intervention: QMHPA utilized active listening skills as Laylah spoke of her current mood. QMHPA aided this intervention with Laylah on social strategies. QMHPA assisted Laylah to behave in more positive ways by addressing the conflict she was facing with her mom. QMHPA will collaborate with mother regarding behaviors and performance in the home. QMHPA will utilize behavior modification system to aid the Laylah with following directions given by authority figures. QMHPA will educate Laylah on behavior modification to help Laylah identify what behavior she needs to decrease and avoid daily. QMHPA encouraged Laylah to have a better day tomorrow. Response: Laylah initially presented with talkative behaviors as she probed the QMHPA and asked different questions about her illness. Laylah later displayed quiet behaviors as she shrugged her shoulders and did not want to answer QMHPA questions. Laylah was witnessed shutting down by mom who called her out right away. Laylah did not respond as information was shared on anxiety and stress. Laylah was assisted in practicing general communications skills. Laylah understood that when she becomes agitated she needs to refrain from taking it out on people and to communicate how she is feeling to others. Response: Laylah reported to QMHPA that she doesnât like the way that people talk to her in the house and that she was not happy being treated like sheâs a bad person. Laylah expressed frustration about this and decided to yell and scream causing a scene during the session. Laylah blames her mental illness on her attitude, mood, and behaviors that she cannot control. Laylah stated that she doesnât intend on being negative and agrees she need to work on controlling and channeling her anger, as well as, listening and communicating her feelings better. Laylah understood how communicating effectively and assertively could help her resist acting out for attention.
